The purpose of the virtual consultation is to provide you an opportunity to learn more about the breast reconstruction procedure you are interested in. In addition, it allows you to share information about yourself and your desired surgical outcomes, helping us determine your eligibility for surgery. Please fill out the information below. Our patient care coordinator will be in touch with you to obtain additional medical information, insurance information, and any necessary photographs. Dr. Tanna will review this information personally and a virtual consultation will be set up that is mutually convenient.
