This 55 year-old female presented with left breast invasive ductal carcinoma. She had a previous history of right breast lumpectomy and radiation therapy. This caused significant preoperative breast asymmetry and distortion, with the right breast much smaller and more deformed than the left. She underwent bilateral nipple-sparing mastectomy and simultaneous deep inferior epigastric artery perforator (DIEP) flap reconstruction.
