This 40 year-old female, with BRCA gene mutation, previously underwent bilateral nipple-sparing mastectomies with implant breast reconstruction by another provider. She was initially told she was not a candidate for flap reconstruction. The patient was unhappy with the implant reconstruction, including the overall appearance (implant malposition, asymmetry, rippling). She underwent removal of the implants and replacement with bilateral deep inferior epigastric artery perforator (DIEP) flap reconstruction.
